Description
As a Case Manager you will play a pivotal role in delivering holistic, high-quality care to clients, overseeing their assessments, developing individualised care plans, and coordinating the services they need to thrive. You will work closely with families and clients to ensure that they have access to all necessary resources and services, ensuring the highest level of advocacy and support. You will be responsible for managing a team of support workers and students, ensuring they are trained, motivated, and provide the best service possible to their clients.
Profile
We are looking for a English and Vietnamese speaking compassionate, dynamic professional with:
• A Bachelor’s or Masters degree in social work, psychology, or a related field
• A minimum of 1-3 years of Case Management experience, ideally in disability services
• A strong passion for advocating for clients and managing a supportive team
• Excellent communication, organisational, and problem-solving skills
• A deep commitment to fostering independent living and achieving client goals
