We're looking for a Support Coordinator who brings empathy, professionalism, and a proactive mindset to champion NDIS participants and help them navigate the supports they need to thrive.

About the Role

As a Support Coordinator, you will work closely with NDIS participants to ensure their individual needs are met through tailored support and service coordination. You’ll advocate on behalf of clients, manage resources effectively, and maintain strong communication with all stakeholders to deliver person-centred outcomes.

Key Responsibilities
  • Client AdvocacyChampion the goals and preferences of your clients, ensuring all services reflect their individual aspirations.
  • Service Coordination – Coordinate health, housing, and social services in alignment with each participant’s care plan.
  • Resource Management – Use NDIS funding wisely and efficiently to maximise positive outcomes.
  • Documentation & Reporting – Keep detailed and accurate records and prepare reports to meet compliance and track impact.
  • Stakeholder Engagement – Build strong, ongoing relationships with participants, families, providers, and other stakeholders.
What You’ll Bring
  • 1–3 years of experience as a Support Coordinator
  • Strong knowledge of the NDIS framework, guidelines, and policies
  • A confident and empathetic approach to advocacy
  • Excellent interpersonal and communication skills
  • Strong problem-solving, time management, and organisational abilities
  • Commitment to client-centred care and outcomes
  • Patience and compassion when working with individuals with diverse and complex needs
Qualifications & Checks
  • Certificate III in Individual Support, Certificate IV in Disability, or Diploma of Community Services
  • NDIS Clearance Check and NSW Working with Children Check (or willingness to obtain)
  • Current and valid Australian driver’s licence
  • Valid Australian working rights
Desirable
  • Experience working with clients who have complex mental health conditions
